Reason Selection: Four radio-style options for why the item is unavailable: 1. "Out of stock" (default selected) 2. "Damaged / expired" 3. "Can't find item" 4. "Wrong shelf location" This data feeds into inventory management and store replenishment systems.

Photo Evidence: Optional photo capture (OPS-P-018) for documentation. Useful for "damaged/expired" reasons or when shelf location is wrong. Dashed border indicates optional action.

Actions: 1. "Find Substitute" (primary lime) — navigates to substitute search (OPS-P-011) 2. "Remove from Order" (secondary outline) — marks item as unavailable and returns to pick list
