Screen Annotations — OPS-D-013 Report Issue

Issue Types: Six predefined radio button options covering common delivery issues. "Customer not available" is the most common and is shown selected by default.
Description: Free-text textarea for the driver to provide additional context. This helps the support team understand the situation and take appropriate action.
Photo Upload: Optional photo evidence to document the issue (e.g., damaged goods, wrong address). Opens the device camera or photo gallery.
Support Hotline: Direct phone link to the support center. Shown prominently in a blue info card so the driver can get immediate help for urgent issues.
After Submission: The report is logged in the system, the support team is notified, and the customer may receive a notification depending on the issue type. The driver is returned to the delivery flow.
